Issue:
On 09/04/13 at 10:57 AM, the Privacy Officer (PO) was sent an email from the Health Information Management Service (HIMS) Chief who is also the alternate PO. The PO didn't read said email until 09/04/13 at 1:45 PM. The HIMS Chief advised that one of the Release of Information (ROI) clerks brought her a letter which they had opened in ROI. It was a letter from Veteran As wife saying that her husband had requested copies of his record, which he received. But included in those copies were copies of Veteran Bs medical records consisting of 4 pages of Radiology and Labs. The information includes his name and address, date of birth, and last four digits of his Social Security Number. The mismailing was sent out by an ROI clerk who is on annual leave until 09/09/13. The HIMS Chief plans to talk with ROI clerk on 09/09/13. The HIMS Chief talked with the ROI clerk who brought her the information of this privacy breach and told her that their outgoing mail must be checked by each other before it goes down to the mail room. When they close the door at 4:00 PM they will check each other's outgoing mail.
Outcome:
09/04/13: Veteran B will receive a letter offering credit protection services.
