Issue:
Veteran A visited the Lee County Medical Center for an appointment. On the way out Veteran A was given a copy of his labs discussed during his visit. The labs were reviewed with the Veteran by a non-VA health care provider when it was realized the records belonged to another Veteran.
Outcome:
05/20/13: An employee form a Community Based Outpatient Clinic gave the Veteran the wrong paperwork. The Veteran will be sent a letter offering credit protection services. 05/29/13: This was determined to be HITECH reportable by VHA Privacy Office.
