Issue:
Veteran A visited the Naples Community Based Outpatient Clinic (CBOC) for an appointment, on the way out Veteran A was given a copy of his lab results discussed during visit. Days later, Veteran A went to review the labs and realized that a portion of the records belonged to another Veteran.
Outcome:
05/20/13: One Veteran will be sent a letter offering credit protection services due to full name and SSN being disclosed. 05/29/13: This was determined to be HITECH reportable by VHA Privacy Office.
