Issue:

Veteran A provided to facility Privacy Officer originals of another Veteran's (Veteran B) laboratory results mistakenly included in same envelope mailed to Veteran A's home address. Laboratory results had been sent by provider to patient to notify of recent results and included name of Veteran B, mailing address of Veteran B, last 4 of SSN, and results of recent laboratory tests. Original of mis-mailed printed laboratory information (one page) was returned to Privacy Officer with explanation of how received and comment that Veteran A "very concerned about this type of mishandling-mismailing of someone else's laboratory results to him."

Outcome:

05/06/14: The Incident Resolution Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
