Issue:

Information containing Personal Identity Verification (PIV) data of a former employee was inadvertently sent to a wrong address by Human Resources (HR). The address it was sent to is one of a current employee who notified our office that she had received it. The information sent included a Removal letter (which contained no PIV data); a Notice of FEGLI Conversion Privilege SF 2819 (which contained the former employees name and DOB); and Agency Letter of Insurance Status (which contained the former employees name, SSN; and DOB).

Outcome:

08/13/14: The Incident Resolution Service Team determined that the former employee will receive a HIPAA letter of notification.
