Issue:

Employee verbally disclosed one patient's PHI/III with another patient.

Outcome:

01/09/15: The employee was involved in an inappropriate relationship with a Veteran patient, and disclosed information between another Veteran patient concerning alcohol addictions and treatment possibilities with the two patients. This employee has since resigned. The Privacy concern was raised when we were processing a FOIA by one of the patients requesting his statement given for a fact finding investigation for administrative actions for the employee. The communications outside of the VA was through Face Book Messenger, texting, phone calls and personal meetings. The Incident Resolution Service Team has determined that two patients will be sent HIPAA notification letters.
