Issue:

A provider handed three routing slips to Veteran A to take to the front desk. One slip was the Veteran's own slip. He handed all three slips to the clerk and said that the provider had asked him to bring Veteran B and C's slips with his to the front desk. Update: 09/18/12:Veterans B and C will be sent letters offering credit protection services.NOTE: There were a total of 28 Mis-Handling incidents this reporting period. Because of repetition, the other 27 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
