Issue:
The medications for Veteran A were mailed to Veteran B and the medications for Veteran B were mailed to Veteran A. It appears the Veterans had the same last initial of the last name. Update: 09/17/12:Both Veterans will be sent HIPAA notification letters.
Outcome:
Medications were retrieved and destroyed. Education was provided to entire pharmacy staff regarding procedures for dispensing medications at the window.
