Issue:
On 01/16/15 at 4:35 PM, the Executive Secretary to the Director sent an email to Privacy Officer (PO) and the Alternate PO. She had taken a phone call from Veteran A's wife who said her husband, the Veteran, received a Summary of VA payments not only for himself but also for Veteran B fom the Mid-Atlantic Consolidated Patient Account Center (MACPAC). The PO didn't read the email until 01/17/15 around 3:00 PM. At 3:45 PM, the PO called Veteran A's wife. She is going to return Veteran B's Summary of VA payments to the PO via snail mail.
Outcome:
01/20/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
