Issue:
A Release of Information (ROI) Clerk in Orlando VA Medical Center Outpatient Clinic processed an ROI request for a patient and accidentally included 3 other patients' medical records in the request. When the requester received his medical records, he noted the packet had 3 other patients' medical records. He then called the Outpatient Clinic to report the incident. The personally identifiable information (PII) that was included on the medical records mis-mailed included the patients' full name, address and last 4 digits of the SSN.
Outcome:
05/21/15: The Incident Resolution Service Team has determined that three Veterans will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
