Issue:
VA Pharmacy employee called PO to report that he accidentally mailed a veteran's insulin to another veteran with the same first and last name (middle initial was different). Error was discovered immediately and veteran who received medication in error was asked to dispose of the medication, which he did.
Outcome:
03/19/14: The Incident Resolution Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
