Issue:
Veteran A received a copy of his claims file and Veteran Bs claims file. There was a problem with the printer, and after it was fixed, the employees thought the cache was cleared, however it was not. Both Veteran A and Bs file printed together. Veteran Bs claims file was very small. It contained his Service Treatment Records, 21-526EZ application, 21-22, notification letter, 4138, share screen, and covers screen. Veteran Bs medical conditions, SSN, and address were disclosed to Veteran A. Veteran A returned the documents.
Outcome:
02/05/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
