A Clinician handed Veteran A Veteran B's clinic router slip. The router contained the full name and date of birth of Veteran B. The Privacy Officer (PO) is unable to determine at this time what other information was on router until the service provides a copy.
Outcome:
06/17/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
