Issue:
Medical records were scanned and sent as a PDF file to the US Air Force. These records were printed and given to the service member. After walking away with the copies the service member returned to inform staff that three pages of the documents belonged to another person. The three pages were given back to the military personnel, who informed the VA of the incident.
Outcome:
10/06/14: The Incident Resolution Service Team has determined that there was a policy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise.
