Issue:
A provider anonymously reported that on 07/3/14 another specified provider had his CISCO phone on speaker, discussing patient care in the hallway on 2S and at the nurse's station. He was talking to Pharmacy at one time and nurses on other floors about the patients that they were caring for. The conversations could be heard by other staff and other patients. He is often seen on rounds standing in the hallway on speaker phone, talking about patient care.
Outcome:
07/15/14: The Incident Resolution Service Team has determined that there was a policy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise.
