Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for one patient (Patient A) when a staff inadvertently faxed Patient As medical information to the wrong recipient.Findings:An interview was conducted on September 21, 2012 with the facility's Privacy Officer. The PO stated that on September 4, 2012, a staff accidentally faxed three pages of Patient A's medical information to the wrong recipient. The PO stated that he became aware of the breach incident when the recipient called the facility to notify them that they received the wrong information.A review on September 21, 2012 of the personal health information (PHI) disclosed in error included:Patient nameCardiology reportThe PO stated that the recipient shredded the medical information. The PO further stated that Patient A was notified of the breach incident.
