Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for 3 patients (Patient's 2, 3 and 4), when a staff inadvertently attached rounds reports to Patient 1's discharge instructions.Findings:An interview was conducted on September 20, 2012 with the facility's Privacy Officer (PO). The PO stated that on September 3, 2012 a staff accidentally attached nursing rounds reports for three other patients to Patient 1's discharge instructions. The PO stated that he became aware of the breach incident when Patient 1 called from home stating there was paperwork attached to his discharge instructions that was not his.A review of the personal health information (PHI) disclosed in error included:Patient nameMedical diagnosisPlan of careThe PO stated that Patient 1 returned all paperwork to the facility. The PO stated that all three patients (2, 2 and 4) were notified of the breach incident.
