Issue:

Veteran A received copies of Veteran Bs Service Medical Record (SMR), Veteran Cs SMR, and Veteran Ds record of Occupational Exposure to Ionizing Radiation. Veteran As file was called in from the Records Management Center (RMC) and was received on station on 04/22/13. Veteran A had two medical jackets which comingled his medical information with Veteran B, C, and Ds. Veteran B, and Ds records are still at the RMC. Veteran Cs record is a VBMS record, located at station 325, with a prior location of station 376. Veteran A returned all of the records to Regional office.

Outcome:

05/23/13: Three Veterans will be sent letters offering credit protection services.
