Issue:

Based on interview and record review, the facility failed to ensure the PHI (protected health information) of Patient A remained confidential This failure resulted in the disclosure of Patient A's PHI to an individual not authorized to receive this information. Findings:An interview was conducted with with the facility's Privacy Officer (PO), on November 7, 2012, at 9:30 a.m. The PO stated Patient A's addressograph (a stamp used on patient's records to identify the specific document as pertaining to that patient) was mistakenly stamped onto the discharge paper work for Patient B.The PO further stated information on Patient A's addressograph reflected the patients name, date of birth and medical record number.A review of the facility policy, "Breach of Patient Privacy: Reporting Requirements (Effective date: 09/23/09)," indicated a breach of health information as, "The unauthorized acquisition, access, use or disclosure of patient protected health information (PHI) that compromises the security or privacy of the PHI."
