Issue:

Based on interview and record review, the hospital failed to provide confidential medical treatment for one of one sampled patient (1). Findings:Patient 1 was admitted to the hospital's rehabilitation unit on 4/25/12. On 10/3/12 at 9:55 a.m. during an interview with the hospital's privacy officer, she stated that part of Patient 1's medical record was given to her office on 8/16/12 by a family member of another patient (Patient 2) who was hospitalized during the same time Patient 1 was hospitalized. The privacy officer further stated that Patient 2 was unaware she had part of Patient 1's medical record for approximately three months and that was the reason why the record was not returned to the hospital sooner. After an internal investigation the hospital identified that Patient 1's paperwork was inadvertently attached to Patient 2's discharge papers and handed to Patient 2 on discharge.On the above same day and time, a review of Patient 1's information that was disclosed to Patient 2 was conducted. The disclosed information included Patient 1's demographic information, history and physical, and medications.
