Issue:

Based on interview the hospital failed to provide confidential medical treatment for one of two sampled patients (1). Findings:Both Patient 1 and Patient 2 had an endoscopy procedure (a procedure which allows a doctor to look inside the human body using an instrument called an endoscope) at the hospital on 8/13/12.On 10/3/12 at 9:55 a.m. during an interview with the hospital's privacy officer, she stated that on discharge, Patient 1 received the discharge instruction for Patient 2. After an internal investigation, the hospital identified that registered nurse 1 (RN 1) had accidently printed the discharge instruction for Patient 2 and handed them to Patient 1. Both patients had the same nurse, physician, and same discharge instructions. The disclosed information included Patient 2's first and last name, and procedure type.
