Issue:

Yesterday (1-25-12) afternoon it was discovered that an Outpatient technician supervisor may have thrown a number of original controlled substance prescriptions in the trash receptacle that were likely in an previously opened UPS envelope that had been received from a CBOC on Friday 20th. The prescriptions had been entered into our processing medication files to filled and marked with a line and annotated as entered. They should have been matched to filling documents and filed after medications packaged and mailed out to patients. The staff has looked throughout the pharmacy area for this envelope and so far, there has been no luck. The veterans names, SSNs, DOB, and medication was on prescription. This affects 11 Veterans. Update: 01/27/12:A letter offering credit protection services will be sent to eleven Veterans.

Outcome:

Aside from notifying the Veterans, we have completed remedial training with staff.
