Issue:

Veteran A reported to Privacy Officer (PO) that she had received Veteran B's lab results included in the mailing she had received at her home address on Saturday, 03/07/15. Veteran A had received 3 pages of laboratory results mailed from VA facility as part of mailed information also containing correctly mailed Veteran A lab results. All 3 sheets of information were immediately turned into facility PO this morning for reporting and necessary corrective action. The laboratory results included recent laboratory test results, full name and mailing address of Veteran B along with last 4 of SSN and name of primary care provider. Veteran A was thanked for returning documents and fact-finding initiated regarding the incorrect mis-mailing of lab results to the incorrect Veteran. Veteran.

Outcome:

03/09/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services. 03/09/15: Correction on last 4 of SSN. The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
