The Power of Attorney (POA) for Veteran A opened his electronic file in the Veterans Benefits Management System (VBMS) to discuss his claim with him, and found Veteran Bs Compensation and Pension exam inquiry form in Veteran A's VBMS file which contained Veteran Bs SSN, list of medical conditions, phone number, home address, dates of military service, disability codes, diagnosis/conditions, date of Birth, first, middle and last name. Veteran As POA is not Veteran Bs POA. The electronic record has been moved to Veteran B's file.
Outcome:
05/21/15: The Incident Resolution Service Team has determined that there was a policy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise.
