Based on interview and record review, the facility failed to ensure that Patient 1's medical information was kept confidential from anyone not authorized access.Findings:An interview with the Privacy Officer (PO) was conducted on 5/30/12, at 1:15 P.M. The PO stated that Patient 1's visit summary was inadvertently given to the wrong patient by a clinic staff. The medical record indicated Patient 1's name, date of birth, date of service, diagnosis, treatment, medications, and medical record number.
