Issue:
Based on interview and record review, the facility failed to ensure that Patient 1 and Patient 2's medical information, remailed confidential from anyone not authorized access.Findings:An interview with the hospital's Privacy Officer (PO) was conducted on 5/30/12 at 1:20 P.M. The PO stated that Patient 1 and Patient 2's medical information was requested by a provider. The PO stated that the transmittal of a patient's medical information was programmed in a computer. During the transmittal of both Patient 1 and Patient 2's medical information, a glitch in the computer occurred which caused the computer to add an extra number. According to the PO, when inspected, the log showed that the fax number inputted in the computer was the correct provider's fax number. The medical records that were inadvertently sent to the wrong fax number indicated Patient 1 and Patient 2's name, date of birth, address, phone number, medical insurance, diagnoses, medical history, medications, laboratory and radiology results.
