Issue:
Veteran A received her FMLA paperwork and included was three forms for Veteran B. Veteran A brought them to the Patient Advocate. The Privacy Officer (PO) happened to be talking with the Patient Advocate about another issue when this privacy issue came up. Veteran B's name, employment information and diagnosis were compromised.
Outcome:
09/18/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter.
