A Veteran called stating that he received three different medications in the mail at home which belong to another Veteran. Outpatient pharmacy is currently investigating the mis-mailing and will notify PO about the outcome.
Outcome:
04/29/15: The Incident Resolution Service Team has determined that three (3) Veterans will be sent HIPAA notification letters due to Protected Health Information (PHI) being disclosed.
