Issue:

A VA Employee was detailed to a temporary assignment. Prior to being detailed the employee's responsibility was to access the electronic medical record of mental health patients to assess the need for psychiatric help. The employee stated he was never given instructions of what to do when he was detailed. While working an assignment he accidently entered a patient record and noticed that the patient had overdosed in an attempt to end his life. The employee shared information with a mental health physician. It was reported to the Privacy Officer that the employee possibly inappropriately accessed patient's electronic medical record.

Outcome:

06/16/13: The Patient will receive a notification letter, due to the patient record being accessed inappropriately and the information disclosed.
