Issue:
Outpatient Pharmacy staff mistakenly dispensed medication to a wrong Patient. Medication contained full name, last 4 of SSN and date of birth of the patient to whom the medication belongs. The med was given to the Veteran at the pharmacy medication dispensed window and about 5 minutes later, he (the Veteran) returned to give it back to the pharmacy staff.
Outcome:
09/25/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
