Based on interview and record review, the facility failed to ensure the confidentiality of Patient A's protected health information (PHI) which resulted in a physician's order to "hold Coumadin "(a blood thinner), was misfiled and sent with Patient B's clinical record when he was discharged to another facility.FindingsOn 7/13/12 at 9:20 AM, the privacy officer was interviewed regarding the report of a possible breach.During the interview, the privacy officer stated, "The physician had written an order on 6/26/12 at 12:47 PM, for Patient A to have her Coumadin held. The order was inadvertently filed in Patient B's clinical record which was being copied and sent to the facility he was being discharged to for rehabilitation."A review of the physician's order sheet for Patient A included:Patient A's name, medical record number, date of birth, physician's name, medications order and a change in the diet.A review of the letter sent To Patient described the events that had led up to the breach of his PHI.
