Issue:

Veteran A and his wife reported to the facility Privacy Officer (PO) that they had received a Radiology CD containing Veteran B's information and had not received the one they requested for Veteran A's own Radiology images/CD. Veteran A returned incorrect CD with the mailed envelope and is upset. He expressed concern and doubt regarding clerical processing and wanted us to find his mailed CD, as it still had not been received at his home address. The CD containing information on Veteran B which was returned by this Veteran A contains labeling on front to include full name of Veteran, full SSN, sex, and DOB along with listing of requested images and dates of exam. Fact-finding to be conducted to determine error in processing and mail-out/necessary remediation-corrective actions.

Outcome:

05/21/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
