Issue:

Veteran A reported to the facility Privacy Officer (PO) that he had received a copy of a faxed list included with his personal photocopied health information which had been mailed to his house. The faxed list was returned by Veteran A to facility PO with a copy of the envelope. The list contained full name and last 4 digits of the SSN on 4 Veterans in which an appointment was being requested by a staff member. Originals of these copies were turned in and Veteran A, who had received the information in the mail, expressed concern re: 'sloppy handling' and wanted to 'make sure issue was addressed and employee responsible should be held accountable.'

Outcome:

12/08/14: The Incident Resolution Service Team has determined that four Veterans will be sent a notification letter.
