Issue:

The Discharge Nurse gave Veteran A the discharge instructions for Veteran B. Veteran A had departed for home before the event was discovered. The Privacy Officer (PO) contacted Veteran A and arranged for the return of instructions by pre-paid mail. Veteran A will return Veteran B's discharge instructions. Until instructions are returned, it is not clear what personally identifiable information (PII) was on the document.

Outcome:

08/11/14: Veteran B's name, full SSN, date of birth and diagnosis were on the discharge instructions. The Incident Resolution Service team determined that Veteran B will receive a letter offering credit protection services.
