Issue:
Noted by County DA's Office that on 8-9-13, disclosure of treatment compliance update was sent beyond the expiration of the authorization created by VA VJO SW and patient. Authority was good through May 2013, but disclosure happened past such date, and was not recognized until VA SW noticed that she could not send another update on 7-10-14, as the authority had expired. VA SW is obtaining a new authority, as Vet is still in program.
Outcome:
07/14/14: The Incident Resolution Service Team has determined that Veteran A will be sent a notification letter.
