An RN at the VA Charlotte Women's Clinic received a telephone call from Veteran A, who reported that when she received her Patient Notification Test Results Letter in the mail this week, in the envelope was also the Patient Notification Test Results Letter belonging to Veteran B.
Outcome:
03/18/14: The Incident Resolution Team has determined that Patient B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
