On March 17, 2014, Veteran A was treated in the Emergency Department, Murfreesboro Campus. Upon return to home, Veteran A noticed he had been given discharge instructions which belonged to Veteran B. Veteran Bs information includes: full name, address, age, and phone number.
Outcome:
03/18/14: The Incident Resolution Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
