Issue:
A Sensitive Access monitor log reported a VA employee accessing a sensitive patient record. The Privacy Officer (PO) requested the employee to provide written justification for accessing the medical record. The employee indicated she accessed the record by mistake and it was her ex-husband's record. The PO will verify from another report to see if another similar name was run that day. Update: 09/28/11:The Veteran will be sent a notification letter.
