Veteran A received a copy of his file, which contained documents pertaining to 3 other Veterans. It is unknown what office provided the copy but it was not BVA. It is unknown if the misfiling was due to a VA employee error or a scanning contractor error. the name and SSN of two Veterans was disclosed. Only the name was disclosed for the third Veteran.
Outcome:
06/08/15: The Incident Resolution Service Team has determined that two Veterans will be sent letters offering credit protection services and one general notification letter.
