Issue:
Veteran A signed for and received Veteran B's medication package at his home address. The package was delivered by UPS, but improperly addressed by Pharmacy personnel. Although the package had Veteran B's name clearly labeled, Veteran A nonetheless signed for the package. When Veteran B questioned the whereabouts of his medication, the error was discovered. The package has been retrieved intact, with no evidence of tampering. Pharmacy personnel mis-labeled the house number as 1211 vice 1221 (same street name).
Outcome:
06/17/13: Veteran B will be sent a notification letter.
