Issue:

Employee's DNR application including disability questionnaire was found in women's bathroom off main hallway of a floor in the facility. VA Physician completed the form, handed it over to an MSA to have it faxed. After fax, application was not immediately disposed of (due to waiting for confirmation of receipt, but still in secure area), and application turned up several days later in a women's bathroom off the main hallway in the facility. After interview with Employee whose information was compromised, determined that he believed that the application was mailed out instead of faxed, and did not receive the physical application back after transmittal. It is unknown at this time why the information was not disposed of after fax transmittal.

Outcome:

10/11/13: The employee will be sent a notification letter.
