Issue:
Based on staff interview and facility document review, the facility failed to prevent unauthorized access to Patient 1's health information when an After Visit Summary (AVS, a summary of a clinic visit) was given to the wrong patient.Findings:During an interview with the Privacy Officer (PO) , on 10/16/13, at 12:17 a.m., she stated an AVS was accidentally given to the wrong patient. She stated the clerk was counseled to check the names on the AVS carefully prior to giving the AVS.During a review of the facility letter document, dated May 14, 2013, indicated the AVS contained the following information:A. Patient's name;B. Medical record number;C. Date of birth;D. Vitals;E. Visit diagnosis;F. Medications;G. Orders for future laboratory tests;H. Date and time of next Physician's appointment.CDPH verified that the facility informed the Department of the Breach within the mandated time frames.
