Issue:

A Veteran's daughter requested a fiduciary update for her father. Based on her request a letter was sent to her address acknowledging the change. But she also received letters for six other Veterans. The daughter returned the letters to the VA with a note about the error. The letters included the full name and full SSN for all six Veterans. It is unknown how the fiduciary was changed for the six other Veterans. This PO is conducting an investigation to find out how the error occurred and if necessary transfer it to the correct PO.

Outcome:

04/30/15: The Incident Resolution Service Team has determined that six Veterans will be sent letters offering credit protection services.
