Issue:
Veteran A called to state that her physician received a disc from Radiology which she had ordered. Upon review, her physician found that the information contained on disc belonged to another Veteran. Also, Veteran A received a copy of a status report, which did not belong to her. She is returning all items to the facility.
Outcome:
04/14/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
