Issue:

The VA Police Department reported a theft of internal documents from the office of Chief of Police. The items missing are candidates' job applications and related documents, which contain applicant name, full SSN, address, phone number, education transcripts, diplomas, DD 214 and additional personally identifiable information (PII)/protected health information (PHI), as well as job vacancy announcements. The missing application packages are dated from the period of January 2011 to April 2014.

Outcome:

06/05/14: The VA Police are currently treating this as a theft. It is not known how many application records were taken, however, it appears to be at least 50. There are cameras in the area but they have not been working. This was reported to OIG on 05/07/14. 06/11/14: The Privacy Officer (PO) has been compiling a list of application packages and the number of affected individuals is close to 500. The PO is going through each package to determine exactly what information has been stolen for each individual. 07/22/14: Privacy Officer is still working with HR and senior management to compile the number of affected applicants. Hope to have a better idea by the end of this week. 08/06/14: The PO reports that 260 applicants information (including SSNs) were stolen. The OIG has stated that VA can send out the credit monitoring letters at this time. All 260 will be offered credit protection services.
