Issue:
Prosthetics Dept. mailed a TENS Unit to a CBOC and included a copy of the patient's consultation in the package. The CBOC clerk handed the package to the veteran. The veteran brought the package to the VA and the staff noticed that the consult belonged to a different patient.
Outcome:
10/30/13: Due to full SSN and medical information being disclosed, Veteran B will be sent a letter offering credit protection services.
