Issue:
The Eligibility Department sent a letter to a Veteran and the SSN was in the window part of the envelope. The SSN was accidently put in where the address should have been. The letter was sent back to the VA as undeliverable and this error was corrected in the patient's record. After reviewing the record it was also found in the Emergency Contact information and this was also corrected by Eligibility.
Outcome:
12/16/13: Veteran A will be sent a letter offering credit protection services.
