Veteran A reported that Veteran B received his clinic appointment letter at home and determined it the letter does not belong to him so he mailed back to him. The Veteran A's full name, home address and protected health information (PHI) was inappropriately disclosed to Veteran B.
Outcome:
05/01/15: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
