Issue:

Patient A's daughter reported that while Patient A was on an inpatient ward, they were approached by Interventional Radio Physician to have daughter sign consent form for Patient B for a specified diagnosis. After refusing (daughter stated Patient A does not have Patient B's diagnosis) the Physician asked for advice from RN presenting on how to have consent signed. RN reported that Patient A does not have the diagnosis for consent. Physician then reported that the board at the front desk has this patient listed as [Patient B's first and last name, with last 4 of SSN]. Physician then left the room without any other words.

Outcome:

03/18/14: The Incident Resolution Team has determined that Patient B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
