Issue:
Veteran A called a VA employee to inform us that she did not have medical records at a specific medical facility. Upon investigation we determined that we requested medical records based on another veterans' Authorization and Consent to release Information form. This resulted in a cover letter showing the PII of Veteran A and attached form of Veteran B.
Outcome:
02/13/14: Veteran B's full name, date of birth and full SSN was disclosed to Veteran A. Based on VA Data Breach Criteria, Veteran B will be sent a letter offering credit protection services.,
